My friend Brent wrote a simple Wiki-clone a while back with the extra feature of an "add" box on every page. The goal was to use a Wiki in something closer to the style of forums and discussion boards while retaining page editing features, simple linking, and the like. I use a version with a few added features (some automatic cross-referencing between pages and automatically generated links to search engines & Wikipedia) for idea tracking and notes.

Both versions can be found at walawiki.org. The code is public domain, and is under active (if low-key) development.
