DokuWiki is a open source web-based wiki. It only requires a web server and PHP.

DokuWiki has many advantages:

DokuWiki localy stores all sites in plain text files - so you can access your data even if the web server is down or non-existent
DokuWiki was designed for print medium - so provides for high-quality printed pages, including footnotes
DokuWiki does not require a big heavy database; only PHP and a web server
